05 GMC drum to disc swap?

I have an 05 GMC that has rear drum brakes. It seems I have to replace the rear brakes and drums about every 1 1/2-2 years. It's a poor design and I'm over it. I want to switch to rear discs. I imagine the easiest way is to swap the whole rearend from a disc equipped truck, or at least swap over the bracketry and rotors/calipers. The other big questions are what else would I need, or need to do? Do I also need the proportioning valve? Is there any computer tuning I need to be aware of to compensate for line pressure?

Any help would be greatly appreciated.
