Brake Line Help

After tearing my truck down 10 years ago to do a ground up with it, I've lost my memory on some things and how they go. Along with ordering newer parts a few years ago on Black Friday specials, I want to make sure I'm doing this right.

WHAT I PREVIOSULY ORDERED
1) Willwood 4 wheel disc brake kit with the attached proportioning valve. Its part number WIL-261-13270-BK.
2) Classic Tubing line kit as attached, part number CST-CT1027-SS.

WHERE IM AT IN THE BUILD
1) Disc brakes all installed.
2) Main line routing complete.
3) Trying to connect lines to the proportioning valve.

QUESTIONS
1) Has anyone else used this proportioning valve? There is an adjustment on the backside that I assume is used to adjust the pressure for the rear wheels.
2) Please confirm, the from port on the proportioning valve is for the front wheels, correct?
3) And the rear port is for the rear, wheels, correct?
4) The brake line kit did not come with a TEE to connect the line of the M/C and then split to the left and right wheels. Anyone know where I can order this TEE?
