Between the cab and the frame you'd have the larger piece, then on the bottom side of the frame mounts you'd have the smaller piece, then the washer, then the nut. The 1/4" between the bushings should be the approximate thickness of the frame mount.

You are correct sir! I was doing them wrong last night. I started doing some research after I posted that and realized what I was doing wrong. I thought one of the bushings went inside the cab like the factory ones. Body mounts are done after a little rust repair! Tranny tunnel is killing me now.
