Here's what I see working with the Tool and Die industry for a while.

The Stamping Die that was producing these was,

Either worn to the condition the punches and Dies needed replacing.

Or the oil flow and air blow to clear the shavings and Scrap was not in Operation,

In the picture I put arrows of things that should have been caught in the stamping process.

With that being said it almost hit 300K.
