Thermostat helps water flow by keeping it in the block longer so you get good thermal transfer. If it's only at idle then it's a fan clutch, belt, or your running lean like chocked. I take it the hoses are not colapsed and your timing is right.

I have not checked the timing but the hoses are new, the radiator has been boiled or cleaned and the thermostat is new. Could it be the guage?

yes, it could be the guage, happened to me recently.... needle would creep up to max hot slowly, like it was really overheating. I pulled the cap and used a meat thermometer to check the temp, it was about 180 but the guage was at max... replaced the guage with a known good one from a spare cluster and now it stays way under half... you might want to check it out...

Is the truck overheating or just heating up at idle? It is normal for a motor to run 15 to 25 degrees hotter at idle than when going down the road. Factory gauges are just to entertain the driver, they are not that accurate. With all factory parts you should be able to get the truck to idle for hours without overheating. My definition of overheating is when it pukes antifreeze out the cap. If it is not coming out the cap it is not hot enought to hurt the motor. From the picture I would say you have the wrong fan. It looks too small for the shroud and it should be a clutch fan.

Tom hand has you covered there, he is right on all of it. If/when you replace your fan, make sure it is not inside the shroud, it should be like 1/2 way in...1/2 way out.
