Yes, it's tough to get to, but with a bit extender it can be done with the engine in.

Here are a few pics of mine. I built everything around moving the engine forward. The only problem I encountered was I had to change the starter to a mini to get clearance for the Speedway ram horns. 700r4 trans 2000 chevy truck driveline shortened, balanced, and all new u-joints. fan shroud and 2 10" fans.

I did my V8 swap many years ago when I was 18 and parked in a friend's driveway. I bought a SBC, Turbo 400 and horseshoe motor mount. I've seen CaptainFab explain in other threads how this set up doesn't work well because that front motor mount does not stop the engine from rocking side to side at all. I don’t remember much but I used the front 6 cylinder motor mount holes because of trans fitment. The truck was not drivable unless I stopped the engine angular movement so I added side motor mounts, welded mounts to the frame (see arrow in photo) and used ¼” plate to bolt them together. It doesn’t look like it would be difficult to find headers that fit but I tried a bunch that didn’t. I have a modified 4 into 1 pipe on one side and a tri-y on the other. If I had more skills and resources, I would have done things differently. Can't say I'd recommend this set up to others.
