That picture makes it look pretty straight. Just a dent in that top rail, but that shouldn't affect much.

Any idea from what could cause the difference in measurement then if the support is straight?

45+ years of living life as a truck can be hard on a truck. There's no way of knowing what is wrong with your truck without an extensive history on it.

look up from the bottom and see if the x-member is straight across or pulled forward. At the tip of the frame rail where the bumper mounts how far across are the rails. Should be really close to 28 1/4''

You could take it to a body shop and have them measure the frame for square. That will tell you a bunch(or if your good with a tape you could do it your self).. From there you can move the cab/ front sheet metal if the frame is square
