6-lug disc brake rotors are either custom bolt pattern or from the OBS (88-98) years.
Since this was some sort of 'kit', I'd put my money on the custom (aftermarket) 6-lug rotor based off of the 73-80 1.25" rotor brake package. As far as the calipers, the lighter GVW 3/4 ton trucks used the same size caliper as the 1/2 ton trucks w/the HD 1.25" brakes.
