What is a good rear end to swap in my 72 c10 to gain 5 lug and disc brakes?

^yes, 72's have front disc, but I think the OP meant rear disc.^

Yeah my truck is all screwed up. I have a 69 frame, I'm thinking about rear disc and maybe going 5 lug. I have a rear end out of a 2003 Silverado. it looks to be about 3 ins wider. Not sure how that will work out anybody done that swap? its 6 lug.
