I think swervin bought some bracket templates off of someone on ebay. His write-up including the templates are here,

A couple of years ago, I talked to the owner running TSM (
http://www.tsmmfg.com) and asked him about the 73-87 rear disc brake kit. IIRC, he told me that they make the brackets, but use the rotors from 88+ trucks by redrilling them for the 5 on 5 pattern. They use calipers and pads from the cadillac. I asked him about having to upgrade the stock master cylinder to a disc-disc master cylinder as some of the other brake companies recommend. He said that it is really not necessary. He said, at the time, they had a contract with a large delivery company and had installed the brake kits on the company's delivery trucks/vans. He said the company never had problems with their kit. (He said the company was in the process of upgrading their fleet with the newer vehicles that come stock with rear disc brakes, so his conversion for this company was no longer needed.)
