The rubber shims are a good idea for between the frame and bed. You can buy a tar fabric material of varying thickness and width in whatever length you want from old car restoration places. I think it's called "anti-squeak". Cut it in 2x2 or 3X3 in. squares and cut a hole for the bolt. This may sound like a backwoods idea but If you have access to old fire hoses, they would make awesome (and durable) cushions for the same purpose.
