This should be clipped to the inner pad and then this clip (with pad attached) should fit tightly within the piston on the caliper. This keeps the pad from rattling around.
These spring clips do wear out and loose they "springiness" so you might need to replace them. Good luck.

I was going to suggest the same thing. I would venture that these are missing and will solve your problems.
