The big difference is that the factory rockers have a rolled edge 1/2"-3/4" wide on the trailing edge. When installed properly (spot welded under the cab), a visible body seam will be present between the rocker/cab and is aligned with the "gap" between the rear of the door/cab.

The aftermarket rockers that I have seen installed do not show the seam. In the day, this was the tell-tale sign of a truck having replaced rockers.
