It turns out that it was an intermittent cam sensor. I still feel something like a slight miss, probably a partially clogged injector. I have put some injector cleaner in the tank so will keep an eye on it.
The truck is much different from my 91. At 55 it is running only 1400 rpm, the 91 is at 1900 for the same speed. This means I need to downshift more as the engine runs out of power sooner. I guess the bigger wheels coupled with a lower differential ratio makes all the difference.
