what kind of shape is it in? and is it the tape on kind or the screw on mount?

It's the screw in type but I think it used factory holes, I'll have to check. I'll get some pictures this weekend and post Monday. It's been in the back of my shop for a while, I remember it being in perfect shape but I'll let you judge from the pictures.
