As for the lug nut you can go down to AutoZone and pick up a set of standard lug nets in a set of 20 with locks for around 25 dollars at least here locally. I am not sure if you can get them without the locks that would be by store I would guess. You can also buy them individually from behind the counter if they have 20 for about the same price as the set with the locks. I think I got some for right at 99 cents but took a while for the store to get in all 20 for the 4 wheels.
The center caps you can get the bolts at a local hardware store. I can't recall the size but I am thinking its 5/16 but that sounds a bit large to me. Depending on where you get the bolts for the center cap ( LMC should have OEM bolts ) as long as the bolt doesn't extend past the wheel you should be able to put the cap on with the wheel on or off the truck.
