Not direct from the factory. Conversion companies did them.

Yes anyone could walk into a Chevy or GMC dealership and order one. You basically ordered a truck the way you wanted it and it was delivered to one of the conversion companies that GM used..at the time I believe it was Scottbuilt, Alton and Pacific Conversions...then they would do the conversion whether it be the crewcab or extended cab and then it was back to the customer. I imagine most were ordered by forestry service and railroads but if you had the money it could be done.
I believe Alton may still be in bussines...I know they were still building crewcabs fo GM up into the 80s.
