more clutch woes

Drove my truck (68 C-10 long bed) to work the other day and the clutch started slipping going over the freeway bridge.I got to work and it had no freeplay.I backed off the adjustment about 1/2 to 3/4 of an inch to get some play.When I tried to start it I got an engine speed tink tink tink sound thankfully it didn't start.I tried again and it locked up.wouldn't crank anymore.The clutch is an 11" Hayes about two years old. previous centerforce lasted about 10 years ;part of the friction disc center cranked and fell out.The sheetmetal around the splined casting broke presumably from u joint angle issues that have been corrected.I can't imagine a piece of the clutch jamming between the flywheel and the bell housing.Anybody got a guess what's wrong?
