Grant wheel with horn cap. This used to be that ugly grey finish and I spray painted it flat black like the black ones. It also comes with chrome screws that make the wheel look really sharp. $45 + shipping.

13.5" I believe. I bought the chrome version at Pep Boys.

Sorry, I didn't see this until now.
You can buy a mounting kit at your local auto parts store. They're around $12. I have a Camaro kit I'd give you that would work, but not correct and would look funny.
