Inserted in rear fender or above step plate.
2004 Mustang GT-R Concept Exhaust Tip shown below.

Stacey David did this on a truck when he was on Trucks, but I can not find any pictures or videos of this.

There is a company that sells through-the body exhaust tips. These certain type through the body exhaust tips are only available from Stylin' Trucks. Perfect for roll pans or straight through fender or your bed.
The through-the-body exhaust tips require a little cutting to install, but the results are worth it according to them.

I called them a year ago and was told, these tips are no longer available.

You might still be able to get something similar.....just look for boat exhaust tips, just be prepared.....they are crazy expensive

I always liked the front exhaust fender exit look. Especially with the SS Header Fender Exit Trim Rings!.
You could shoe horn a muffler in there some where
