You could adapt the GMT-400 style shifter that bolts to the driver's side of the hump to your truck. A friend of mine put a square body transfer case in his GMT-400 for his SAS and built a z-bar type pivot that bolted across the trans/tcase adapter which allowed him to shift the tcase from the other side of the tunnel.

Here is an NV-4500 and NP-241 that is going into a 1988 GMC K1500, the passenger side drop NP-241 is from a square body. Notice the linkage for the transfer case.




It is built for this style shifter.
