You only need to vacuum the system out if you have opened it up or the charge in there has gone to zero. If zero though you should fix the leak, which means opening it up and now you need to vacuum.
If you're topping up, no need to vacuum. That said, R134A is not as forgiving to under and overcharge. For systems from the factory as R134a, it is recommended to evacuate the system and charge by weight.
