I had started a thread about this several years ago and to build one from scratch the easiest way would be start with a crew cab chop the doors down x amount of inches then shorten the cab. Cut the b pillars weld them to the back doors and then find some suicide hinges for the back doors to open like a regular extended cab would.
Install a 90s or 2000s rear extended cab seat and modify the stock door panels to look decent. Also to shave the rear door handles and install the ones into the b pillar so they open like current extended cabs. Then either shorten the frame to run the stock bed or build a flatbed for it. But shortening the frame would also include a new/ modified rear driveshaft. My plan was and still is if I find a crew cab that is clean enough is to shorten it just enough to get rid of the window in the back doors that stay stationery then the body line around the window will still match the truck unlike the ones that were build from the coach companies It would definitely be a long build to make everything look nice
