Thanks to a 4 yr old thread & a post by Tx Firefighter, it looks like the u joint needed is indeed the Precision 372.

Now the wild card is whether or not the driveshaft will work as-is. The 12 bolt is still under my truck, so its not easy to get an accurate measurement from the axle tube centerline to the centerline of the yoke thats on it.
