Cruising the depths of marketplace for a local small block for a rebuild and I saw this. Ad says it's a 350 from an 80s pickup and I wondered why there was only 1 accessory hole in the heads. Then I saw the backside and I'm guessing they swapped sides with the heads for whatever reason or is there a breed of cylinder heads that are supposed to be like this? It's been a while since I rebuilt an engine

I believe that came from a 86-87 truck or van. 1pc rear main seal, 193 heads and 210hp.

That is an 86 to 92 350. You will need to use an electric fuel pump. During casting they blocked off the fuel pump hole. I know the BB of that time did not. I have an 87 engine and this is how I know. Also the center bolt valve covers.

86-89 you could still get a mechanical fuel pump in the R series. It needs to be checked if wanting to run the mechanical pump.

I say its 86-87 because it doesn't have the lifter valley bosses cast into it.

As far as I know there has NEVER been unique left or right cylinder heads for any SBC. They all fit either left or right. On a used engine it's always possible one of the heads has been swapped for a different casting number or model year.
