How do you re-create an options sheet?

Does anyone have a file or know how to re-create a glove box options sheet? mine is all tore up and would like to have a nice looking sheet to have on the glove box door....I don't know if you can do this or if it is even worth spending my time on (I feel like it is) but i am sure i am not the only person ever to think of this. (I am not that smart)

Thanks

Rick

Here is a file I have on my website made by a prior boardmember http://yukon.b0xen.us/misc/option.xls
If I remember correctly, this file can be modified in Excel or a Corel spreadsheet.
