clunk clunk..when shifting gears.

After letting the clutch out while shifting gears I hear and feel a CLUNK coming from the drive train. I had the both yokes repaired and drive line, and that did help some. The question is? How often does the problem lie in the trans/transfer case coupler vs the rear posi? how hard is this to repair this coupler? any advise is most welcome. Thanks.
