Bolt pattern and backspacing?

I am looking for a new set of wheels for my truck. Its got 5 bolt rallys on it now, but how do I measure the bolt pattern? Is it center to center of two bolts across from each other? Also, what backspace or offset should I look for and how wide? I will just be getting 15", likely slots or something. Thanks!

This may help with the bolt pattern. Rear back spacing is usually from 4.75" to 5.00"
