Help on Painting

I know someone out there knows about painting a vehicle so here it goes. The truck I'm working on has rust thats pitted down in the metal a little in places, especially the top. Do you have to completely sand it down to where there is no dark places in the metal, or can you leave the dark places and primer over it, then paint it? I sure could use the help.
