I have a version you can type and reprint yourself with Microsoft Excel. If you PM me an e-mail I can send you the file.
Also the board vendors make a duplicate sheet that you can retype them yourself.

Seems like board member chipflyer sells them. He's the only one I have seen able to reproduce the correct font.
