Collecting some crew cab info

Just for curiosity's sake, I guess, I'm looking for any information you guys have on crew cabs from our period. I realize most were created for logging/rail industries, and that none (or one???) were created by the factory.

What I'm looking for is pictures of period-correct crew cabs, as well as the companies that built them (I'm not looking for modern-made ones). I know Scott-Bilt was a big player in that market, and I'm roughly familiar with at least one Pacific Coach conversion. Any sales information, brochures, pics, etc. would be greatly appreciated.

Also, were any created using Burb doors out back rather than modified truck ones? Or did some not even have a back door, but a folding front seat?
