I recently acquired a 1969 K2500 fleetside 4 x 4. It has a 454 big block with a NP 208 transfer case and turbo 400 in it. The truck currently has a 4 inch body lift on it and a 4 inch suspension lift. My plan was to increase the suspension lift, and decrease the body lift. While I was under the truck today checking clearances I noticed the transfer case was only 1 inch below the cab. The engine and transfer case are all mounted properly but I am confused as to why there is only 1 inch of clearance over the transfer case with a 4 inch body lift. Has anybody on this combination (454/NP208)? Is it normal to have to lift the body to clear the transfer case?

Is it a high hump cab or low hump? It sounds like a 2wd (low hump) cab on your truck.
If that's the case you can cut the low hump out, and bolt a high hump in.

If that's not it than it probably has something to do with the transfer case. The NP208 wasn't stock in this body style.

How can I tell if it is a low or high hump? Does anybody know the measurements?

I did some reading on the hump. My question is does the hump make for transmission clearance or also the transfer case clearance?

Both. The high hump was used for 4spd transmission and all 4wd. They bolt in and the low hump aren't.

If you can't eyeball the low hump vs high hump just pull the carpet back. As Tim says the low hump is stamped into the floorpan, it is part of the pan. The high hump bolts onto the florr pan.

Confirmed it is a 2wd cab. At least I know what the problem is. I am going to keep the body lift as I dont feel like cutting it up right now!
