door hinge backing plate r&r

I assume the only way to get access to these is to cut a hole either in the pillar or inside the cab? I have an upper that has broken loose from what ever keeps it from falling backwards and wanted to be sure I wasn't missing something before I started slicing my cab up.

That's correct. You'll have to cut in order to gain access.

If you can still get to the plate and bring it back up where it should be you could remove the paint and find the spot welds that held it on. Drill them out then use short bolts to hold the backing plate in place and plug weld where you drilled. Its a lot more finicky but would save trying to refit the cut out section.

Well now that I have it cut out, now what? Naturally I did not cut out enough of the pillar to have room to fix and reattach the rusted out bracket. Anyone have a suggestion on how to re engineer this?

You need to cut out a large enough section of the pillar to bench repair then reinstall. Your cuts should be at least a couple of inches above and below the hinge bolt holes in order to leave enough material for the little brackets that retain the backing plate.
