You'll find the beehive style (70-91),but those 69-only (hockey puck) are rare to find. They are the exact same lens,but clear,as reverse lights on Stepsides,cab & chassis,flatbeds,and StepVans. If you find those you could pint the insides a color that matches the "amber" plastic.

I'm going to go outside here in a minute and take a picture, but the reverse lamp lens is not the same as the hockey pucks that I have?

My '69 had hockey pucks originally, that lens was included in the box of NOS parts that I got with it and is the only one left. It has the aftermarket Fireman's hats on it, so far as I know they were installed when it was repainted in the late 80s or early 90s.

The reverse lens will, in fact, mount in place of the roof lens, the diameter and screw locations are the same, but the sealing surface is different.

Replacement (age/origin unknown, possibly GM) markings - the number on the underside which you can see to the bottom right is 684662, which is the GM part number for this lens

I am sure glad you posted the pictures with the info because I need 5 of the Signal Stat 9069A lenses for a 1975 C-30 I just bought and all the places that I have found want quite a bit for them. I found them by doing a search and found them on a Ryder Truck Parts website for about one third the cost of other places.
