I know this is old but that is a power brake booster. They are independent of the master cylinder so they are usually mounted on the driver side fenderwell.

These heavy duty truck engines were never designed for speed, they are designed for strength, developing high torque at low rpm's. Note large distance between cylinders and how block extends below crankshaft area.

The bigger 401- 478's were found in these types of trucks. A 401 GMC V6 puts out 348 lbs ft of torque at 1600 rpms, a "400" Chevy big block puts out 350 lbs ft of torque at 3000 rpms. The big block Chevy has to spin almost twice as fast as the GMC to put out the same amount of torque to get loads moving.

And today’s beloved Gen III LS small blocks have to spin 4 times as fast to squeak out that kind of torque

Those ole GMC V6’s were a hoss. Slow as a snail but they could pull down a city block
